Vision 360 enables collaborative care in Greenwich
NHS Maritime Doctors LLP is the latest federation of GP practices that has chosen Vision 360 to share information between GPs, enabling collaboration and consistency of care.
NHS Maritime Doctors LLP (Maritime Doctors) is the federation that covers the Woolwich and Plumstead areas of Greenwich CCG. All twelve practices in the federation use Vision hosted on AEROS, including the local Clover Health Centre based in Woolwich town.
The federation has an immediate requirement to improve its facilities for managing long term medical conditions. They have operational targets to achieve for patients with Diabetes, Heart Failure, COPD and Hypertension. In particular they need to increase the number of patients being reviewed for these conditions and ensure consistency of care planning. To achieve this, specific clinics will be instigated at different locations; any patient will be able to attend any of these clinics rather than having to visit the practice where they are registered.
GP practices simply cannot provide these additional services in isolation and therefore need to collaborate. Maritime Doctors has ordered Vision 360 to facilitate shared working and empower GPs across the federation by giving them the ability to view and add to a patient’s record, regardless of which practice the patient is actually registered at. It also allows appointments for the long term condition clinics to be centralised and booked by reception staff at any of the federation’s member practices.
INPS will begin by creating the shared appointments system so Maritime GPs can offer shared long-term condition clinics at their practices. Patient record sharing will be introduced when the clinics go live. When these services are up and running the solution will be expanded to include out-of-hours appointments, increasing the number of appointments available for patients at the weekends and in the evenings.
The final stage will be to introduce tailor-made data entry screens for the long term clinics, using the same templates as those used within the Vision systems at the GP practices to encourage consistent care.
Maritime Doctors is the latest addition to the rapidly increasing number of federations using Vision 360 to share patient records and appointments between GPs. Although all practices within Maritime Doctors use Vision, other federations are using the same solution to allow GPs to read and write to patient records across a combination of Vision and EMIS Web systems.
